Natalie Gibson: The nag personal trainer/Personal Trainer Client / Trainer

Agreement

The nag personal trainer will use her skills and knowledge to design a safe programme of exercise that will take into account the personal goals, fitness levels and exercise likes and dislikes.  The nag personal trainer will provide the coaching, supervision, advice and  support that the client may need to help them achieve their goals. The client’s progress will be regularly monitored and the programme revised and adjusted accordingly.  I understand that although my Personal Trainer may include recommended caloric ranges, macronutrient distribution, and suggested foods to ingest (or,  in some cases, avoid) to better meet my fitness goals, my Trainer is not a  nutritionist or registered dietician (RD), and she is legally not allowed to  prescribe a specific meal plan for me. I understand that any specific diet limitations need to be discussed with my GP, nutritionist, or Registered Dietician to ensure proper monitoring. All information provided by you will be kept confidential and will not be made available to third parties until permission is granted to do so by you. If the trainer requires further medical information from a practitioner, the client must provide such details.  It is understood between client and trainer that both will commit and give the programme and 100% effort.  The client should arrive 5 minutes prior to a training session so that a full session can be achieved on each visit.  The client is required to wear appropriate clothing and footwear for physical activity.  I realise that participation in the program includes but is not limited to exercising, use of exercise equipment and strenuous exertion (strength training) all of which increase heart rate and body temperature. I understand that exercise involves certain risks, including but not limited to, serious neck and spinal injuries resulting in complete or partial paralysis, heart attack, stroke or even death. Also, injuries could occur to bones, joints, or muscles. Slips, falls, and unintended loss of balance could result in muscular, neurological, orthopaedic, or other bodily injury. I understand that part of the risk involved in undertaking any activity or program is relative to my own state of fitness or health (physical, mental, or emotional) and to the awareness, care, and skill which I conduct myself in that activity or program. You are free to deny or withdraw consent at any time after consenting. Knowing the material risks and appreciating, knowing. and reasonably anticipating that other injuries are a possibility, I hereby expressly assume all of the delineated risks of injury, all other possible risk of injury, and even risk of possible death, which could occur by reason of my participation. It is your full responsibility to inform The Nag Personal Trainer of any difficulties you perceive or experience as well as changes to your physical and medical condition. You consent to being aware of your own health and physical condition and having knowledge that your participation in The Nag Personal Trainer’s programme may be injurious to your health and you are voluntarily participating in The Nag Personal Trainer’s exercise programme.  I do hereby waive, release, and forever discharge The Nag Personal Trainer from any and all responsibilities, or liability for any present and future injuries or damages resulting or arising from my participation in any activities including but not limited to use of the nutrition guide, exercise, personal training or use of the equipment including any injuries and damages caused by the negligent act or omission of any of those persons or entities mentioned above.  

Personal Training Terms and Conditions Health Screening:

All clients must complete a PAR-Q before commencing any exercise programme.  Your trainer may require a letter of ‘medical clearance’ from your GP. Please be aware that your GP may charge you for this.  Fairness Policy In the event of cancellation, the following applies:

Package payment clients

 If you give less than 24 hours’ notice the session will be lost. Less than 48 hours’ notice, I will aim to find another session within the next 7 days.  If I am unable to do so, the session will be lost. 72 hours’ notice, I will aim to fill the session.  If I am unable to do so, I will attempt to find another slot within 7 days of the original session, however if I am unable to do so, the session will be lost.  I will notify you with at a minimum of 24 hours’ notice if a slot is available.

Pay per session clients

If you give less than 24 hours’ notice, the full session fee will still be due. Less than 48 hours’ notice, I will aim to find another session within the next 7 days.  If I am unable to do so, the full session charge will be due.  I will attempt to fill the slot, in which case, there will be no further charge.72 hours’ notice, I will aim to fill the session.  If I am unable to, I will attempt to find another slot within 7 days of the original session, however if I am unable to do so, the session will be lost. I will notify you with at a minimum of 24 hours’ notice if a slot is available. The nag personal trainer has the right to suspend or cancel any further sessions if she feels it is required or appropriate to do so after full discussion with the client.  Lateness Policy:  If the client is late for the training session the session will not be extended and will end at the appointed time.   If the nag personal trainer is late for a booked training session, the client will be granted another training session with no charge.    

Group Personal Training Policy 

To participate in group personal training, you must be able to do all the dates decided by the nag personal trainer.  All the sessions are to be paid 48 hours before the first session starts, otherwise the space will be given to another client.  The client must complete a health and lifestyle questionnaire to participate; otherwise, they will NOT be allowed to participate in any of the sessions.  If the client is late, they will not be allowed to into the session. This causes disruption for the other clients, and they need to be warmed up safely to take part.  The client is to be at least 5 minutes early to the session and ready to start.  If the client misses a session, they will NOT get that session back or be credited.  If the nag personal trainer has to cancel the session, that session will be set up for another date.   If the nag personal trainer is late, the session will still be 1 hour long.    

Fitness Classes

Class booking special offers are for one person only. If you are booking 4 or more classes and receiving a discount, please only add one place per booking for each session to your order. If you book multiple slots for the same class and time slot the booking will be cancelled, and a cancellation fee deducted from the refund amount. We appreciate that errors occur, if you have booked multiple slots by mistake send us a message and we will swap you to an alternative class and/or time slot without charge. 

All participants of any exercise class organised by The Nag Personal Trainer must be aged 16 or over and must have completed a Physical Activity Readiness Questionnaire (PARQ). If in the opinion of the nag personal trainer taking the class, you are not able to take part in the class due to the questions answered on the questionnaire you may be refused access to the class and may be referred to your GP. PARQ’s can be completed online prior to attending or can be completed with the trainer taking the class. Please allow extra time to complete a PARQ on your first visit to a class. It is the responsibility of each user to bring to the attention of the instructor any changes which may cause the user to answer YES (when previously answering NO) to any question previously answered in any PARQ.

Fee Charging Policy:

Training plan fees are to be paid on a monthly pre-pay basis or by full block session payment. When paying for personal training sessions by standing order the minimum number of monthly payments is 3. After 3 monthly payments it is the client’s responsibility to cancel the direct debit if they do not wish to continue. All transactions will be in UK Pounds. Payments are made through PayPal. It is advised that you refer to their terms and conditions for further information on how your data is stored, used and processed. Monthly payments are per calendar month. Payment for single sessions must be made at the time of booking. Cheques can be made payable to Miss Natalie Gibson, standing order (request bank details) PayPal.Me:thenagptfitness or by cash. Package bookings must be paid for in advance BUT sessions do not have to be booked in advance. However, all sessions must be redeemed within the timescale agreed between you and the nag personal trainer (group personal training sessions excluded).  All monies paid are non-refundable.  The nag personal trainer reserves the right to revise and/or update these terms at any time.  Rates and prices can vary at any time at the discretion of the nag personal trainer.   These terms form your agreement with the nag personal trainer and form a legal agreement between us so please read and understand them.  The nag personal trainer reserves the right to vary or update these terms at any time. Your continued use of this service constitutes your acceptance and agreement to the terms. I recognise and understand all the terms and conditions set between my personal trainer and myself and agree to follow all the guidelines set out above.  I hereby grant permission to the nag personal trainer the rights of any photographs or videos of me, without payment or any other consideration.   Photographic, audio or video recordings may be used for marketing and promotional purposes within (but not limited to) commercial presentations, online platforms, Social media posts By starting personal training with the nag personal trainer, I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in the public business settings. I will be consulted about the use of the photographs or video recording for any purpose other than those listed above. I further acknowledge that I have completely read and fully understand the above release and agree to be bound thereby.  If you do not grant the permission to the nag personal trainer as detailed above, please inform the nag personal trainer in writing or by email.
